Please click on the building-specific link below to register your student for their flu shot. You will be asked to select an appointment time – this time does not matter, we will call students as we are ready for them. (Hy-Vee’s scheduling system requires that time be selected. If there are no appointment times available please reach out to the school nurse.) We suggest registering your student using your smartphone, as you will need to take a photo of their insurance card.
High School – Wednesday, Sept 22 – beginning at 8:30am

If you would like your student to receive the COVID-19 vaccine, you will need to print and fill out the attached COVID vaccine form. Your student must be 12 years of age or older on the date of vaccination to be eligible. This form must be returned to the school nurse by Monday, Sept 20. (Students are able to receive both the Flu Shot and the COVID-19 vaccine on the same day.)
As always, these vaccinations are voluntary, your student is not required to receive the flu shot or the COVID-19 immunization.
We encourage you to reach out to your student’s healthcare provider for questions or concerns about these immunizations.
